Clean Hands Self-Certification Form
A completed and signed Clean Hands Self-Certification attests that the business does not owe the District of Columbia $100.00 or more.

DC law requires all businesses applying for a Basic Business License to be compliant with the DC Office of Tax and Revenue.
File an (FR-500) Combined Registration Application for Business DC Taxes/Fees/Assessments with the DC Office of Tax and Revenue (OTR) online, via mail, or in-person at 1101 4th Street, SW, W270, Washington, DC 20024. After registration, a Clean Hands Certificate (also known as a Letter of Good Standing) can be obtained from OTR. In lieu of a Clean Hands Certificate, the BBL applicant may complete a Clean Hands Self-Certification.
A Clean Hands Certificate must be submitted at the time of BBL application or renewal.
